*Appropriate documentation MUST include diagnosis, prognosis, and functional limitations.*
Office of Academic Success/Disability Services cannot authorize any accommodations unless it is supported by documentation. The documentation determines which accommodations the student may be eligible to receive. In accordance with federal and state laws, this documentation must include each of the following:
- Statement of disability, including DSM IV where appropriate, and that clearly identifies the condition.
- Statement of prognosis: permanent or temporary (how long).
- Statement clearly defined limitations related to the disability. Note these limitations must effect a major life activity (which includes learning) as established by the ADAA 2008 amendments.
- Statement list or identify treatments, medications (including any side effects), assistive devices or accommodations that the student could benefit from.
- All documentation must be signed and dated by an identified diagnostician on official letterhead, with the contact information for the individual providing the diagnosis.
- All documentation should be current (no older than three years old) unless it is a condition that is static such as vision or hearing loss.
- Office of Academic Success/Disability Services will not accept documentation hand- written on physician prescription pads, notes on random paper or other types of paper which are not official letterhead.
