Incoming Freshman and Transfer Students

The following forms need to be completed and returned BEFORE you are eligible to compete in athletics at the University of Minnesota Morris. These forms need to be sent to the Athletic Training department together to the following address:

Health History and Physical Examination: complete before your physical and bring to your physician appointment. Return to UMM following physical.

Athletes have the option of having their own physician perform a physical, or an on-campus physical can be completed free of charge.

Pre-Participation Physical Examination: bring to your physician appointment and have physician complete and sign. Return to UMM following physical.

Emergency Contact and Insurance Information: complete and return to UMM.

Drug Testing consent form: complete and return to UMM.

Returning Athletes

Insurance

To be in compliance with NCAA legislation all student athletes must have proof of personal insurance. The NCAA recommends that insurance coverage should have a limit of $75,000 and cover athletically related injuries. If you do not have insurance please contact the Head Athletic Trainer and a policy will be made available to you. This policy is at a cost of $90 for the year and covers only athletically related injury. If your insurance company requires care to be delivered at a specific site or by a specific provider (gate-keeping), you may consider changing your provider for your child to one of the local physicians during the academic year, and then back to your original provider during the summer. This allows for care to be provided locally with less travel time. A list of local family physicians is available by contacting the Head Athletic Trainer.

Drug testing

UMM has an institutional random drug testing policy that is administered twice a year throughout the entire program. Random testing will be held once in the fall and once in the spring. All athletes are subject to be tested whether in season or not.
