The form can be downloaded and completed in Microsoft Excel format. Please do not submit hand written forms. Completed forms should be emailed to ummstact@morris.umn.edu or delivered to the office of Student Activities, Conferences and Special Events in the Student Center by the deadlines shown in the schedule below.

Hearing Schedule for 2011-2012

Request forms may be picked up in the Office of Student Activities or downloaded from the AFRC website and will be due by 4:30 pm in the Office of Student Activities on the following Thursdays. Organizations requesting funds will be contacted about the a hearing time.
