Request For Network Connection/Disconnection/Reconfiguration Service

Type of network request: Ethernet-twisted pair connection - billable
Ethernet-twisted pair disconnection - non-billable

All installations, disconnections, and reconfigurations of the network will be billed at $100/connection.
