Release of Information
Disability Services requires documentation from your doctor or health care professional in order to provide services and accommodations (see Disability Documentation). To allow communication between the DS office and parents, agencies, counselors, or service providers on campus, a signed Release of Information form is required.

Release of Information to Disability Services

This form is used to request documentation of a disability from a medical or counseling facility where the disability was diagnosed.

Release of Information between Disability Services and other parties on and off campus

This form is used to provide consent for the UMM Disability Services staff to communicate with other campus service providers, instructors, and/or the parents of a student as specified on the form.

Release of Information by Disability Services to another party

This form should be used when a student, faculty member, or staff person with a disability is requesting information on file in the UMM DS office to be copied and sent to another party such as a transfer college or graduate school.
