In order to receive services from the Disability Services Office at UMM, you must complete an application form and provide documentation of your disability.

Submit your completed application and documentation to:

You may also email a copy of your completed Application for Services as an attachment to Colleen Frey or Bob Smith in the Disability Services office. The Information Release Form requires a signature and may not be submitted electronically.
