Health Services in collaboration with the Student One Stop Office at the University of Minnesota, Morris (UMM) would like to announce the availability of summer health service for UMM students at Stevens Community Medical Center (SCMC) from May 13, 2013 to August 27, 2013. Students registered for classes during the spring semester of 2013 or the summer of 2013 are eligible for this medical appointments service.

To access a medical appointment:

• Please stop by or contact the Student One Stop Office at 320-589-6046 for information about the Stevens Community Medical Center Voucher. This voucher is good for one clinic or urgent care medical appointment at Stevens Community Medical Center. The voucher must be approved by UMM Student One Stop prior to the clinic visit and presented at the SCMC appointment desk at time of the appointment. Further appointments/urgent care clinic visits will each require another voucher.
• Contact Stevens Community Medical Center to schedule a clinic visit or visit the urgent care clinic during posted hours. SCMC clinic and appointment desk hours are available at: http://www.scmcinc.org/clinicservices.htm.
• Present your Summer Health Service Stevens Community Medical Center voucher when you check in at the appointment desk.

Students are responsible for scheduling their clinic appointments or using urgent care hours. This voucher covers only the cost of the appointment. It must be presented at the time of the appointment or urgent care visit. All other associated medical costs are the responsibility of the student. This voucher does not cover emergency room visits.

The Health Service office will be closed from Monday, May 13 through Friday, August 9, 2013. Though the office is closed, calls or emails sent will be addressed in a timely manner.

Health Service will open with regular hours beginning Monday, August 12, 2013.
