Withdrawal Forms

Withdrawal: Discretionary, One Time Late Drop (PDF)	- Used when a student wants to drop a course after the 9th week of instruction or equivalent, but before the last day of instruction for the term
Withdrawal: Non-Academic Reasons (PDF)	- Used when a student needs to withdraw from a course due to extenuating circumstances AFTER the 9th week
Withdrawal: After the Last Day of Instruction (RTF)	- Used when a student wants to petition to withdraw after the last day of instruction

Dean's List Petition (PDF)	- Used for exceptional circumstances
Dean's List Endorsement (PDF)	- Used when grades are turned late by faculty
Change of College (PDF)	- Used when Morris students want to transfer to another U of M campus
Health/Hospitalization Insurance Form (PDF)	- Provide information about private coverage for staff to enter on your record
Non-Degree Application for Enrollment	- Non-degree seeking/non-U of MN students can use this to indicate their intent to take classes at UMM, including summer session coursework
Petition Form (PDF)	- All-University form that can be used to request almost all academic record exceptions, alterations, or changes
Readmission Application (PDF)	- Used when you want to return to UMM and continue taking classes, or transfer classes back to UMM to graduate
