Drop Classes, Cancel ALL Classes, Withdrawal Forms

Withdrawal: Discretionary, One Time Late Drop - Used when a student wants to drop a course after the 9th week of instruction or equivalent, but before the last day of instruction for the term
Withdrawal: Non-Academic Reasons - Used when a student needs to withdraw from a course due to extenuating circumstances AFTER the 9th week
Withdrawal: After the Last Day of Instruction - Used when a student needs to petition to withdraw after the last day of instruction for non academic reasons

Grades and Degree Forms

Incomplete/Course Completion Agreement - A formal agreement between a student and instructor when an "I" grade/incomplete is given. See form for more info.
Degree Application apply online - go to GRADUATION
Duplicate Diploma Request Form - Graduates of the University of Minnesota may order multiple copies of their diploma, which will be mailed to you for the prices listed on the form

Miscellaneous Forms

Area of Concentration (major) - Link to Dean's Office website
Dean's List Petition - Used for exceptional circumstances
Dean's List Endorsement - Used when grades are turned late by faculty
Change of College (IUT) - Used when Morris students want to transfer to another U of M campus
Information Release (SIRA) Used only by staff in the Office of the Registrar, Financial Aid and/or Student Accounts Receivable - see Parent/Guest access for on-line access.
Late Registration Fee Waiver Appeal - Effective Spring 2010 students must register before the first day of class
Non-Degree Student Request to register - Non-degree seeking students can use this to indicate their intent to take classes at UMM, including summer session coursework
Petition Form - All-University form that can be used to request almost all academic record exceptions, alterations, or changes
Readmission Application - Used when you want to return to UMM and continue taking classes, or transfer classes back to UMM to graduate
Special Exam Form - Must meet with Scholastic Committee Representative first, office location: 204 Behmler Hall
Special form for administrators from students who wish to grant access to discuss private data with parents
